Dear employee,

Welfare and health of employees is a complex matter and therefore Revan N.V. has decided to contract Medwork Caribbean N.V. as per June 1st 2023. Medwork is a company which offers services regarding Occupational Health, Insurance Medicine, and HealthCare Management Advisory through its specialized team of healthcare professionals.

Instructions for reporting work disability

Please view below the adapted procedures for reporting work disability. Employee(s) must follow all the below stated procedures.

1. On the first day of work disability (AO), the employee must report via telephone to his/her direct supervisor/manager at least [1 hour] before the initiation of work. Ensure to personally talk to him or her. If reporting to SVB is applicable to the employee see point 2 otherwise skip to point 3.

2. The employee must now report to the SVB via telephone number 527-2782 or online. This number is available Monday through Friday from 7:00 am to 12:00 noon and from 1:00 pm until 4:00 pm and on weekends and official holidays from 7:00 am until 11:00 am. If this is not done the work disability (AO) will not be agreed upon.

3. The employee must now report to Medwork to receive a sick leave consult. The Sick Leave reporting to Medwork is done by phone or Whatsapp:

Each employee will call/msg Medwork directly according to the following schedule:

  • Reporting by Phone (Monday-Friday); +297 5878880.
    Calls are only taken from; 8.00am – 12.00pm / 1.00pm – 5.00pm
  • Whatsapp SickLeave Report Line +297 7330072 is available during the following hours only from Monday-Friday: 8.00am – 11.30am / 1.00pm – 4.30pm
  • Whatsapp SickLeave Report Line is only available for messages to report sickness. No calls are accepted on this number. Should an employee want to speak with one of the representatives, they should call the regular telephone line: +297 5878880.
  • Should the employee get sick during his/her shift after 5.00 pm (during a weekday), he/she will have to report directly by phone or Whatsapp at 8.00 am the next day.

Procedures for reporting (AO) on weekends and holidays

  • During the weekend and holidays the schedule to receive calls (+297 5878880) will be 8 am to 11 am, for consultations on the same day.
  • Should the employee get sick after 11.00 am during weekends and holidays, the person needs to report by phone at 8.00am the next day.
  • No Whatsapp messages are accepted during weekends and holidays.

4. When calling in sick at Medwork, the employee will be given a time for the appointment with the Company Doctor which should be followed strictly otherwise a No-Show report will be sent to the employer.

5. If an employee is declared able to work by the company doctor, the same day, the second day or the third day, but the employee does not feel fit yet on this day, the employee will have to follow the steps from the first point again.

Important points


The employee who is disabled to work needs to report this personally which mean that no one else can make his call for the employee. Missed calls, sms’s, voice mails, e-mails are not allowed.

Consequences for not adhering to work disability instructions.
The consequence for not following the new sick leave procedure is that the sick leave will not be paid.
